Getting Started

Before you submit your complaint, please help us by having all the information about your concern ready to go. If you have documents you would like to include you can upload them at the end of the complaint submission process.

Please be sure to include all the details of your complaint so that we can help you as quickly as possible. Your complaint will be forwarded to a staff member with the Bureau of Long Term Care.

There are five things to identify in your description when submitting your complaint:

  1. What is the complaint about?
  2. What type of problem are you having?
  3. What happened?
  4. Who is the complaint about?
  5. Who is affected by the incident?
If your complaint is NOT about the Duals programs please click the Begin… button below.

If your complaint is about the Medicare Medicaid Coordinated Plan (MMCP) or the Idaho Medicaid Plus program (IMPlus), you must first contact the health plan that is administering the program.
